Use any image editing program to make 128x128 size png files with transparency.
Then use iconworkshop by axilas to convert them to icon files.
For best results, export each different size of icon seperately.
I think cuc means to create them from scratch. Like Photoshop or something.
Easy, make a shape. Select the shape and create two layers each with gradients going in the opposite directions. Gaussian Blur the bottom gradient with a nice large number. Set that layer to Screen. Create a circular marquee doubled the image size and offset the selection and delete it from the top gradient creating a glare. On the main picture, add a innerglow with Normal and about 2-10 pixel width with black color. And a dropshadow at 90 degrees at 40% opacity. Got it?
