I'm experiencing the same thing, albeit only sometimes...
I imagine it depends a lot on what modifications you've made to your system: I'm using FindeXer, QTTabbar and QT Breadcrumbs address bar in Windows Explorer, so I guess this is slowing down the system somewhat...
Maybe someone here has a few hints on a remedy for this, but it might help if you give a few details of the system you're running, i.e. XP/Vista, what mods you're using etc... 
If this happens all the time even after a fresh boot then you may need to clean up your context menu.
If it only happens after long time use than you need to reboot or kill explorer and restart it. Reboot is the more stable way.

search for some registry entries... i guess google can help you

geeq is right, you'll have to edit the registry for specific items.
You could try Fast Explorer.
It will let you clean up static entries or at the very least disable other ones.
