Help me make sexy nurse icon

Hello icon community.

I want to turn this nurse picture into an icon for my virus program. I am not the author of this image but have received permission to use it for these purposes. At the moment I have made the icon but the transparency has these crappy rings of white which looks crappy.

EDIT: The white circle has disappeared after I restarted windoze. The only thing I need someone to do is fix the transparency on the bottom of the chair and the bottom of her stiletto.

How can an icon not be vista ready?

A Vista 256 px compressed icon format cannot be created by IconWorkshop using Windows XP. A true Vista icon contains that format. The comment is meant as a "HeadsUp" rather than a "It won't Work".
