It looks like Samurize but, could be done with SysStats.
Also, I believe he's a member here so maybe a pm might be better to find what you're looking for?

Hmnn, I'm guessing Informer for Yahoo Widgets and Itunes bar for AveTunes, though it could be Samurize. Using a smilar combo at the moment
