How does the "Shuffle" feature work?

I (along with I assume millions of people) have always been quite curious as to how "shuffle" works on mediaplayers.. My only thought was it takes the time+date or a series of numbers (1 to ∞) and than works off preset or random multiples... The smarter ones I (again) assume would work off of third party factors like playcount, lastplayed, date added, ect, ect. Anyone know?

itunes has a built in shuffle slider that allows you to set exactly how likely 2+ songs from the same album are played in a row. I don't think playcount, rating or time/date played factor in on most media players.
