that's an os x app called menuet, which adds all sorts of functionality to iTunes.
I take it, however, that you are looking for a windows version? And that is where i cannot help.

the controls could be achieved with Avedesk or samurize or any widget app on Windows. The lil icon on the systray is however impossible on Windows as it is dynamic i.e. it changes accordingly with the playing song..
