How do you go about making skins for iTunes?

Is there a certain program or something?

You need to have the Multi-Plugin for iTunes see here. After installing that you go into iTunes preferences, select Multi-Plugin, go to the Other tab and hit Dump Resources.

In the iTunes skin folder you will find a folder named Developer Template. C:Program FilesiTunesSkinsDeveloper Template

What you do now is draw over every single bitmap with your new skin graphics until you have a complete skin. The best way to do this I've found is to take a screencap of your iTunes in full and mini mode, load them in Photoshop and make a mockup of your skin. Then you can just cut/paste/edit as necessary for your skin. One nice thing about it is you can do it while iTunes is operational. So you can change a bitmap, reload the skin, and see your changes live instead of having to reload iTunes every time.

It's not simple, it's not easy, it's not fast. But you will succeed if you just take your time and have fun with it. Enjoy ^^.
