Go to Control Panel (User CP on the forum) and Edit Avatar, then Use Custom Avatar.

Remember: The maximum size of your custom image is 100 by 100 pixels or 20.0 KB

There are plenty of PSDs available. Susumu released a PSD for the t-shirt I'm using. I added the color and logo. You will need Photoshop, however.

PSD are basically Photoshop Documents; like there's .docs for Word, .xls for Excel, etc.
