Windowblinds can do this, I believe, although I don't use it myself. I've been searching for a registry hack for a long time to enable this, and I know it must be possible, because somewhere there must be an instruction to tell Windows to grab the exe's icon.
I've not found it yet, however, so the best advice I can offer you is:
With reshack, you can replace firefox.exe's icon with an invisible icon instead. You can use this method with any executable for this effect. I've attached an invisible icon to this post, although making one, obviously, is very straightforward, just thought it might save you some time. Just extract the zip, it's the only file in there and will be suitable for any replacement.
This will fix the titlebar icon and the taskbar icon as they're drawn from the same place.
As for folder windows in explorer, I can't offer any help, although I'll continue to search for some kind of solution in the registry. Maybe someone else will be able to offer some insight into how this might be achieved.

I had already thought of this method but the problem is *as you say* for the folder windows icon in explorer... I don't know if this is possible or not but it's a pity not to be able to hide this icon.

i have a solution to the firefox icon thing without changing the exe file....all you do is place a blank .ico in
C:Program FilesMozilla FirefoxchromeDefault and name it main-window.ico

You have to purchase, but just use the blank icon provided up there ^^^. Though, I can't work out if its remembering the settings or not (I am using trial only).

Finally, I found an other way. For the explorer icon, I changed the 16x16 icon format for my folder icon and apply it. So the icon is correctly displayed in 48x48 or 64x64 and for the 16x16 format in the taskbar, it shows my custom icon (which can be transparent)
