There are countless of downloadable free scripts online - google for web form script...

Some require the server to have email, some not. Depends on your needs...

http://www.planetsourcecode.com has some good user submitted scripts, in any language. You can only view 5 pieces of code a day, or you need to resister (free). It's a very good source.
