[help] How Can I Create A Feedback Form?

Hello!

I am in the process of creating my first website in Dreamweaver, having used Front Page and notepad before.

On my website I wish to create a feedback page. I have tried to do it for about an hour or two now and I have gotten nowhere. The form just goes to the thank you page without sending me the form results via email.

Can someone please tell me what is the simplest way to make a form in Dreamweaver MX 2004 that sends you the results via email?

I just realized that my web hosting company has Jack's Form Mail Script installed on their servers. Is that easy to use? Does anyone know a tutorial which could tell me how to make a feedback form using this program?
